Routine Home Care

 

Faced with a terminal illness, 90% of patients prefer to receive care at home, surrounded by people they love. This is the most common level of hospice care, with core services provided by the hospice team.

Continuous Care

 

Intended to support patients and caregivers through brief periods of medical crisis. Skilled care may be brought into your home to alleviate severe pain and symptoms.

In-Patient Care

 

Required when a patients’s pain and symptoms become unmanageable. Patients may be transferred to a facility to receive additional care.

Respite Care

 

Short-term care that relieves the primary caregivers (family, friends, loved ones). Patients are temporarily transferred to an in-patient facility or nursing home.

Your total peace of mind is assured with the knowledge that a registered nurse is available 24 hours a day, and 7 days a week. The nurse will deal directly with a crisis situation, answer questions, and visit the patient, as needed.
